29 CFR 1926.454(b): The employer did not have each employee involved in erecting, disassembling, moving, operating, repairing, maintaining, or inspecting a scaffold trained by a competent person to recognize any hazards associated with the work in question. Reference 29 CFR 1910.27(a) (a) Establishment, warehouse - On or about September 30, 2023, an employee was operating a Genie GS-3232 Scissor lift from the outside of the machine causing him to run over his foot. The employee was not provided with practical training and operators performance evaluation prior to operating the scissor lift.
