OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:  a) Imperial Bag & Paper Co. LLC located at 350 4th Street, Perrysburg, Ohio, 43551: On or about October 19, 2023, employees tasked to perform work from the Yale (Model: 0S030EAN24TE089) high lift order picker truck were not benefited with an adequate means for fall protection.  Employees utilizing the picker truck to retrieve and place products on tiers of the racking systems were exposed to falls approximately 18 feet to the concrete floor below.

LLC located at 350 4th Street, Perrysburg, Ohio, 43551: On or about October 19, 2023, the employer failed to ensure employees were protected while performing work that required employees to stand on pallets of the tiered metal racking system.  Employees  access the elevated levels of the metal racking system, without the benefit of effective fall protective systems were exposed to falls of approximately 18- 25 feet to the concrete floor below.

LLC located at 350 4th Street, Perrysburg, Ohio, 43551: On or about September 19, 2023, the employer failed to ensure employees had been trained on topics to include: recognition of fall hazards and utilization and maintenance of fall protection devices, prior to performing work from elevated surfaces. Employees tasked with work atop the various tiers of the metal racking systems with out the benefit of this training were exposed to falls of approximately 18-25 feet to the concrete floor below.

LLC located at 350 4th Street, Perrysburg, Ohio, 43551: On or about October 19, 2023, the employer failed to ensure that a hazard assessment identifying and selecting appropriate personal protective equipment was conducted prior to employees performing warehousing activities such as, but not limited to, accessing inventory of the metal racking systems, operating powered industrial vehicles and utilizing the Paper Cutter or Edge Saw in the cutting room.  Employees performing warehousing activities without an employer assessment were exposed to bodily injuries from falls, falling objects, flying particles and struck by hazards.

Employees tasked with performing service and maintenance tasks such as, but not limited to, changing blades of the Harris Seybold Saber IV Power Back Gauge Cutler Hammer (Paper Cutter) without the benefit of energy isolation, hazardous energy recognition training and employer evaluation, exposed employees to amputation hazards created by the unexpected energization of the equipment.

Power industrial vehicle operators and pedestrians were exposed to potential struck-by and crushing hazards in that employees operated the vehicles without the benefit of training.  b) Imperial Bag & Paper Co. LLC located at 350 4th Street, Perrysburg, Ohio, 43551: On or about September 19, 2023, the employer failed to ensure that each employee tasked with operating powered industrial vehicles such as, but not limited to, the Yale "parts picker" were adequately trained through a combination of formal instruction, practical training and evaluation prior to operating the powered industrial vehicles throughout the warehouse.  Power industrial vehicle operators and pedestrians were exposed to potential struck by and crushing hazards in that employees operated the vehicles without the benefit of training.
