29 CFR 1910.184(e)(3)(ii): The employer did not maintain a record of the most recent month in which each alloy steel chain sling was thoroughly inspected and shall make such record available for examination. On or about October 19, 2023, and at times prior thereto, the employer did not conduct or maintain inspection records for alloy steel chain slings. Employees were exposed to struck-by and/or crushed-by hazards.

29 CFR 1910.212(a)(3)(ii):The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. On or about October 19, 2023 and at time prior, employees of Maxey Trailers manufacturing were exposed to working with a hand held grinder without a guarding device attached.
