29 CFR  1910.132(d)(1):The employer did not assess the workplace to determine if hazards were present, or were likely to be present, which necessitated the use of personal protective equipment: a) Maintenance shop: A workplace hazard assessment on insulating the outer lining of the furnace was not performed to determine if hazards were present, exposing an employee to inhalation and dermal injuries from contact with refractory ceramic fiber. b) Production: A workplace hazard assessment on bagging and handling Vermiculite, and Perlite products was not performed to determine if hazards were present, exposing employees to respiratory diseases.

29 CFR  1910.134(k)(6):The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer: a) Therm-o-Rock Perlite Byproduct yard bagging area: An employee who voluntary wore an N95 respirator during bagging of Perlite by product, had not received a copy of the information contained in Appendix D prior to use, exposing the employee to hazards from the improper use of the respirator.

29 CFR  1910.253(b)(4)(iii):Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour:  a) Maintenance Shop: Oxygen cylinder and Acetylene cylinder stored adjacent on welding cart with no fire resistance separation subjecting employees to fire hazards.
