On the roof of a temporary commercial worksite, located at 1765 Memory Lane, Jacksonville, FL 32210: On or about October 18, 2023, the employer Universal Roof & Contacting failed to ensure its sub- contract employees 5 Star Roofing Services, LLC protected its five employees from eye injury hazards caused by airborne debris and flying objects, in that eye protection was not utilized by the employee, while using pneumatic nail guns to install roofing shingles.
