29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns.   At the construction site, the employer did not ensure that employees were wearing hardhats while working below other employees. Employees were exposed to a struck-by hazard from falling objects.

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  At the job site, employees were not provided eye protection when operating pneumatic nail guns and were exposed to eye injuries from flying objects.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.  At the construction site, employees were working on a jack scaffold without ladder access. The employees performing siding installation activities were exposed to a fall hazard.

29 CFR  1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.   At the construction site, employees were working on a jack scaffold without fall protection. The employees performing installation of siding were exposed to a  fall hazard greater than 10 feet.

At the construction site, employees were working on a jack scaffold without fall protection training. The employees performing siding activities were exposed to being struck by material falling from above and fall hazards.
