29 CFR 1926.502(e)(9):Positioning device systems were not inspected prior to each use for wear, damage, and other deterioration, and defective components were not removed from service: Site- An employee operating a Genie S-60 aerial lift was not protected from an ejection hazard where the Werner self-retracting lifeline used as part of a positioning device system was not inspected for damage prior to use and removed from service where damage was evident. The Werner self-retracting lifeline did not retract the cable all the way back to the device, approximately 3 feet of slack would not retract into the device. The locking function of the Werner self-retracting lifeline would not engage during inspection. The identification, inspection, and user labels on the Werner self-retracting lifeline were mostly all removed and not legible.
