29 CFR  1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:   Site- Employees using an aluminum extension ladder to access the roof were not protected from a fall hazard where the top of the ladder did not extend three feet above the landing surface.

Site - Employees working from roof were not protected from falls between approximately 18 and 25 feet where fall protection was not required to be utilized by the employer.
