29 CFR 1926.24: The employer did not develop and/or maintain an effective fire protection and prevention program at the job site throughout all phases of the construction, repair, alteration or demolition work: (a) On or about October 16, 2023, two contracted employees cutting and welding a lifting eye onto compressor skid 1 were exposed to fire hazards in that the employer did not have an effective fire protection and prevention program at the job site throughout all phases of the alteration work.

29 CFR 1926.25(b): Combustible scrap and debris was not removed at regular intervals during the course of construction: (a) On or about October 16, 2023, two contracted employees performing cutting and welding work within the main compressor building at the Danville Compressor Station were exposed to a fire hazard when compressor skid 1 had combustible debris in the form of miscellaneous petroleum product in, on, and around it that was not being removed at regular intervals while preparations were undertaken for the compressor skids removal.

29 CFR 1926.25(c): Garbage and other waste were not disposed of at frequent and regular intervals: (a) On or about October 16, 2023; and at times prior thereto, employees and contracted crew walking and working within the main compressor building at the Danville Compressor Station were exposed to a slipping hazard when miscellaneous petroleum product that spilled on the dirt around the compressors was being partially captured with spill pads, but the pads were not disposed of at frequent and regular intervals.
