(b) Production - On or about October 18, 2023, the employer exposed employees to burns while working on a line where candle wicks are set with a junction box cover left open while line was in operation.

29 CFR 1910.36(d)(1): Employee(s) were not able to open an exit route door from the inside at all times without keys, tools, or special knowledge: a) Production and Storage - On or about October 18, 2023, the employer exposed employees to entrapment hazards, in that, exit doors were equipped with a lever style door handle and deadbolt installed. b) Shipping - On or about October 18, 2023, the employer exposed employees to entrapment hazards, in that, double door exit doors were equipped with a lever style door handle and deadbolt on one door installed and the other was locked using a security bolt on the top and bottoms of the door.
