This was most recently documented on or about October 19, 2023 at 5078 South 111th Street, Omaha, Nebraska. The employer failed to conduct workplace hazard assessments to determine if personal protective equipment was necessary and the types of personal protective equipment that would protect employees from chemical hazards. Chemicals used in the facility included but were not limited to Photoreactive Resin, EBECRYL 8210 Radiation Curing Resins and isopropyl alcohol.
