29 CFR 1910.146(c)(3):When the employer decided its employees would not enter permit spaces, the employer did not take effective measures to prevent its employees from entering the permit spaces: a) 515 South 1st Street East, Malta, MT: On or about October 17, 2023, and at times prior, employees were exposed to entrapment hazards while working and walking around multiple open permit required confined spaces. The employer failed to take adequate steps to ensure the employees could not access these areas.
