The rotating pipe/tube was turning while employees held sandpaper against the pipe to remove gouges.  b.  On or about October 19, 2023, the employer did not ensure one or more methods of machine guarding were provided to protect the operators and other employees from point of operation and rotating part hazards while working on the finning lathe. There was no chuck guard or guarded work zone to contain processed materials, fluids, parts, and tooling within the work zone.
