On or about October 17, 2023, during the installation of roofing material at the job site located at 6219 Dora Blvd, Independence Ohio 44131; the employer did not ensure employees working on the roof of a residential structure were protected from possible danger of eye injury from impact, or from flying objects from the employees installing roofing material.
