Caution shall be exercised when handling off-center loads which cannot be centered. a) On or about 10/19/23, the employer did not ensure that employees had safely arranged or stabilized an approximately 6000lbs industrial conveyor frame on the forks of the a Mitsubishi Forklift (Model FG30N, SN:AF13G04375) in that the forklift was used to upright the conveyor and the forks were elevated to hold the conveyor in place so that employees could clean out the conveyor before it fell and trapped an employee.
