29 CFR  1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the jobsite, materials, and equipment to be made by a competent person(s).  Location: 54 Union Street, Attleboro, MA  On or about Oct. 19, 2023 Employees were exposed to fall hazards where no fall protection was in place, and employees were exposed to struck-by hazards from overhead work, falling construction debris and tool, where hardhats were not worn. The employer did not provide a competent person to perform frequent and regular inspections of the site.

29 CFR  1926.501(b)(1): Each employee on a walking/working surface having an unprotected side or edge which was six or more feet (1.8m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest system   Location: 54 Union Street, Attleboro, MA  On or about Oct. 19, 2023 Employees were exposed to fall hazards, where fall protection was not used.  Instance 1: Over 50 feet from a 5th floor rooftop, to the pavement below  Instance 2: Over 20 feet from a roofline, to the pavement below  Instance 3: over 20 feet second level/ barn door, to the pavement below
