On or about October 23, 2023, at the warehouse, exit access on aisleway #1 and #2 were obstructed with pallets, ladders, shelving and merchandise exposing employees to fire and entrapment hazards.   b.) On or about October 23, 2023, in the store, the exit access on the aisleway along the back wall was blocked with merchandise, exposing employees to fire and entrapment hazards.

On or about October 23, 2023, at the warehouse, the exit door on the right was blocked on the inside with boxes and on the outside with two pallets of merchandise, exposing employees to fire and entrapment hazards.   b.) On or about October 23, 2023, at the store, the exit door located at the back left side was blocked with a ladder and shelving materials, exposing employees to fire and entrapment hazards.

On or about October 23, 2023, at the rear of the warehouse, fire extinguisher next to the emergency exit door located on the right side, was covered by boxes and other trash, exposing employees to fire and entrapment hazards.  b.) On or about October 23, 2023, near the entrance to the warehouse, a fire extinguisher was not properly mounted and was found sitting on the floor, exposing employees to fire and entrapment hazards.
