29 CFR 1910.37(b)(2): The employer did not ensure that each exit was clearly visible and marked by a sign reading "Exit": a) On or about October 23, 2023, and at times prior, the employer did not ensure that the exit doors in the warehouse were not marked with "Exit" signs. This exposed employees to the unsafe condition of inadequate exits.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket): a) On or about October 23, 2023, and at times prior, the employer did not ensure that plug connected equipment including a sump pump which had been used to siphon hot tubs, were inspected before use for external defects. The sump pump had damage to the outer jacket which had separated from the pump housing exposing the cord's internal insulated wiring. This exposed employees to the electrical hazards.
