29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toe boards, safety net systems, or personal fall arrest systems: a) 615 East Main Street Palmyra PA, Jobsite Roof - On or about October 23, 2023, Employees performed roofing activities on a one-story restaurant with a 6:12 roof pitch without fall protection and were exposed to fall hazards up to approximately 12 feet. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladders were not used at an angle such that the horizontal distance from the top support to the foot of the ladder was approximately one-quarter of the working length of the ladder. a) 615 East Main Street Palmyra PA, Jobsite: On or about October 23, 2023, employees were performing work on a commercial roof approximately 12 feet 7 inches from the asphalt to the roof edge. Employees used a 20-foot ladder extended to an approximate working length of 15 feet. The foot of the ladder was located approximately 7 feet from the bottom edge of the wall that the top part of the ladder was braced upon. Corrected during the inspection, no abatement certification or documentation required.
