29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye: a) Work site - On or about October 17, 2023 and at times prior thereto, the employer did not report the hospitalization of an employee until 3 days following the incident. The Van Hook Services Co., Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1904.39(a)(2), which was contained in OSHA inspection number 1567574, citation number 1, item number 2 and was affirmed as a final order on March 1, 2022, with respect to a workplace located at Mirabito Energy 302 Route 6, Milford, PA 18337.
