29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:   The employer was failing to protect employees from fall hazards. This was most recently documented on October 24, 2023 at a jobsite located at 4205 South 223rd Plaza Circle, Elkhorn, Nebraska when employees were observed working on a residential structure. The employees were installing shingles while working at a height of approximately 12 feet 6 inches measured from the ground to eave without fall protection.

29 CFR 1926.1053(b)(1): The employer is exposing employees to fall hazards where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   The employer was failing to protect employees from fall hazards. This was most recently documented on October 24, 2023 at a jobsite located at 4205 South 223rd Plaza Circle, Elkhorn, Nebraska when employees were observed working on a residential structure.  A ladder was used by the employees that was not extended over the upper landing surface.
