29 CFR 1910.134(c)(2)(ii): The employer did not establish and implement those elements of a written program necessary to ensure that any employee using a respirator voluntarily was medically able to use that respirator.  a) 1 JLG Drive, McConnellsburg, PA 17233 - on or about October 24, 2023, employees were using 3M Speedglas Welding Helmets equipped with an Adflo High-Altitude Powered Air Purifying Respirator (PAPR) Assembly (Part number 7100214375).  The PAPR was provided by the employer for voluntary use.  The employees had not been evaluated to ensure that they were medically able to use the respirator.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
