29 CFR  1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  On or about October 4, 2023, employees were exposed to caught-in hazards when changing the blade on a band saw without�an established�energy control�program.

29 CFR  1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a. On or about October 23, 2023, employees were exposed to caught-by hazards when operating a DoAll band saw that was not guarded to protect the operator from the unused portion of the saw blade.  b. On or about October 23, 2023, in the back of the Manual Machine Department, employees were exposed to caught-by hazards when operating a Roll-In band saw that was not guarded to protect the operator from the unused portion of the saw blade.  c. On or about October 23, 2023, in the middle of the Manual Machine Department, employees were exposed to caught-by hazards when operating a Roll-In band saw that was not guarded to protect the operator from the unused portion of the saw blade.  d. On or about October 24, 2023, employees were exposed to caught-in hazards when ingoing nip points on a Porter-Cable belt sander's bottom pulley were not guarded.  e. On or about October 16, 2023, employees were exposed to caught-by hazards while using a Giddings and Lewis Bickford radial drill press without a guard to protect employees from the rotating parts.
