29 CFR  1926.454(b): The employer did not have each employee involved in erecting, disassembling, moving, operating, repairing, maintaining, or inspecting a scaffold trained by a competent person to recognize any hazards associated with the work in question.  a) On October 23, 2023, at the jobsite, where the employer did not provide training that prohibited employees from standing on the guardrails of the JLG Scissor lift.

29 CFR  1926.501(b)(1):  Each employee on a walking/working surface having an unprotected side or edge which was six or more feet (1.8m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or person.  a) On or about October 23, 2023, at the jobsite, where the employer did not provide fall protection for employees installing a heating, ventilation, and air conditioning (HVAC) unit on the roof of a new building structure.
