29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered only under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration: a) Job site, 7607 McKnight Road, Pittsburgh, PA: On or about October 24, during masonry activities, the employer did not ensure that the scaffolding was erected by a competent person. Deficiencies noted during the inspection included, but were not limited to, no safe access on/off the scaffold, working levels not being fully planked and not having adequate guardrails systems in use when working at a height of at least approximately 12 feet.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards: a) Job site, 7607 McKnight Road, Pittsburgh, PA: On or about October 24, during masonry activities, the employer did not ensure that an employee working on a scaffold was trained in scaffold safety where hazards existed. Deficiencies included, but were not limited to, the lack of ensuring safe access on/off the scaffold, working levels not being fully planked and proper guardrail systems not being installed for fall protection when working at heights of at least approximately 12 feet.
