29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a) Airstream Inc. on or about October 16, 2023, at the address of 1001 West Pike Street, Jackson Center, OH; The employer did not ensure that energy isolating procedures for the Komo CNC Routers were implemented prior to employee performance of cleaning activities. Employees tasked with cleaning activities on and around the CNC's automated table while in an energized state were exposed to caught-in hazards.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. (a) Airstream Inc. on or about October 17, 2023, at the address of 1001 West Pike Street, Jackson Center, OH: The employer failed to ensure the nip-points created between the automated table and the fixed frame on the Komo CNC Router was adequately guarded. Employees performing operational activities at or around the nip-points of this machine were exposed to crushing hazards.
