29 CFR 1926.759(a): All materials, equipment, and tools, which are not in use while aloft, were not secured against accidental displacement.: Location: a) Worksite; 4th floor to 1st floor; northeast zone a) On or about 10/24/2023, Employees were walking/working on the first and second floors, which were below an uncovered hole. An object, identified as a DOKA H2O beam, fell through the hole on the fourth floor and struck two employees on the head.
