29 CFR 1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. On October 05, 2023, the employer did not report the in-patient hospitalization of an employee that suffered a broken clavicle and a fractured skull as a result of a fall from approximately 10.6 feet within 24 hours. The employee was hospitalized for the incident and has not returned to work yet. OSHA was not notified until October 20, 2023.
