29 CFR 1926.501(b)(4): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: Worksite located at 3455 W. 76 Country Blvd., Branson, Missouri- Employees were exposed to fall hazards created by the removal of sheet metal that created floor holes in which the employees could fall through. The employees were observed working without the use of lanyards attached to their harnesses.
