(b) On or about October 25, 2023, inspection of the North Ammonia Refrigeration Engine Room, identified that P&IDs did not reflect current piping configuration associated with pressure relief valve connecting C-10 oil cooler to C-10 Oil separator.  The valve was marked as TWG valve.  (c) On or about October 25, 2023, inspection of the North Ammonia Refrigeration Engine Room, identified that P&IDs did not reflect current piping configuration in that the valve was not labeled or identified on the  P&ID.

29 CFR  1910.119(l)(1): The employer did not establish or implement written procedures to manage changes to process chemicals, technology, equipment, and procedures, and changes to facilities that affect a covered process.   (a) On or about October 25, 2023, a management of change (MOC) was not implemented for Cyrus Shank pressure relief valve that connected the oil cooler and oil separator on Compressor C-10 that was beyond the five year manufacture required replacement date. The Cyrus Shank pressure relief valve (75 psig) was installed in December of 2011, was past the required replacement date by 6 years.  (b) On or about October 25, 2023, a management of change ((MOC) - 52) was not completed before commencement of installation of new spiral freezer in the North Ammonia Refrigeration Engine Room.  The employer commenced work on the new spiral freezer for the north ammonia system without the completion of MOC-52. The work commenced on June 2023 and the MOC was not completed till October 2, 2023.   (c) On or about October 25, 2023, a management of change was not initiated and completed before commencement of installation of coil addition for the new spiral freezer in the North Ammonia Refrigeration Engine Room.  The employer commenced work on the new coil addition for the north ammonia system without the initiation of MOC-53.
