29 CFR  1926.451(b)(1)(i): Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) was not installed so that the space between adjacent units and the space between the platform and the uprights were no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary.  Location: 1736 80th Street, Brooklyn, NY.  On or about 10/25/2023,  a) Employees worked from a scaffold platform that was not fully planked or decked.  One area was with a single plank, the space between the platform and the upright was approximately 14 inches.

29 CFR  1926.451(b)(2): Scaffold platforms and walkways were not at least 18 inches wide.  Location: 1736 80th Street, Brooklyn, NY.  On or about 10/25/2023,  a) Employees worked from a scaffold platform that was form by a single plank.  The width of the platform was approximately 10 inches.

29 CFR  1926.451(c)(1)(iii): Ties, guys, braces, or outriggers were not installed to prevent the tipping of supported scaffolds in all circumstances where an eccentric load, such as a cantilevered work platform, was applied or was transmitted to the scaffold.  Location: 1736 80th Street, Brooklyn, NY.  On or about 10/25/2023,  a) Employees worked from a scaffold that has not been adequately secured against tipping.  The scaffold was tied to the wall by tie wire.

29 CFR  1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Cross-braces were used as a means of access and egress.   Location: 1736 80th Street, Brooklyn, NY.  On or about 10/25/2023,  a) Employees did not have a safe means of access to the supported scaffold.  CSHO observed employees descending from the scaffold by climbing the frames and cross braces.

29 CFR  1926.451(g)(1)(vii):Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  Location: 1736 80th Street, Brooklyn, NY.  On or about 10/25/2023,  a) Employees working from a scaffold exposed to fall hazards of approximately 12'.  One section of the scaffold did not have any guardrail.
