29 CFR  1926.451(c)(2)(iv):Front-end loaders and similar pieces of equipment were used to support scaffold platforms without being specifically designed by the manufacturer for such use:   Worksite located at 3315 S. Hammons Blvd., Joplin, Missouri- Employees were exposed to a fall hazard when they got into a platform supported by  a front-end loader that was not designed by the manufacturer for the use of this platform.

29 CFR  1926.501(b)(1):Each employee on a walking/working surface with an unprotected side or edge 6 feet or more above a lower level was not protected from falling by the use of a guardrail system, safety net system, or personal fall arrest system:   Worksite located at 3315 S. Hammons Blvd., Joplin, Missouri- Employees were observed working and walking on a 4th story deck level and were not protected from falling by the use any means of fall protection.
