This was most recently demonstrated on or about October 26, 2023, and at times prior to, when wood dust and fibers were generated as part of the chipping process. This exposes employees to the potential of fire and explosion hazards. a) General-purpose electrical fixtures, conduit, and wiring methods were used and installed in a Class 1 Division 2 location in the mill areas. The air hose was attached to the hose fitting by a worm drive-type hose clamp which was not designed for use on an air hose. The clamp was associated with the Rotunda air compressor and thereby exposing employees to possible whipping action of the hose if the clamp were to come off.

This was most recently demonstrated on or about October 26, 2023, and at times prior to, when portable cord and plug sets and flexible cord sets were being used with visible damage. This exposes employees to potential electrical hazards. a) A flexible cord set (extension cord) was being used by employees during breaks to prepare meals and was found to be missing a ground pin in the lower/downhill mill. b) A cord and plug wall fan was being used by employees to ventilate the workspace and was found to have visible damage to the outer jacketing; as well as internal damage in the lower/downhill mill. c) A flexible cord set (yellow extension cord) was used by employees to power an overhead light and was found to have visible damage to the outer jacketing in the pallet shop. d) A flexible cord set (orange extension cord) was used by employees to power an overhead light and was found to have visible damage to the outer jacketing in the pallet shop.
