On or about October 25, 2023 at the address of 799 Elizabeth St. Lima, OH 45804: The employer did not ensure each affected employee used appropriate eye or face protection while utilizing pneumatic nail guns during roofing work activities on residential structure. Employees working in this condition were exposed to potential struck by hazards (flying projectile/ricochet nails).

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a)	Sky Buildings LLC. On or about October 25, 2023 at the address of 799 Elizabeth St. Lima, OH 45804: The employer did not ensure employees were protected from falls while performing roofing work activities (asphalt shingles install) atop a two-story single family residential structure with a 6 -12 roof pitch. Employees operating in these conditions were exposed to falls of between 8ft - 14ft to the ground below.
