29 CFR  1910.28(b) Protection from fall hazards -  (1) Unprotected sides and edges.  (i) Except as provided elsewhere in this section, the employer must ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the following:  a) On or about October 24, 2023, the employees were exposed to a fall hazard of 6 feet 10 inches due to unprotected sides and edges at the Giddings & Lewis Horizontal Boring Mill Model H70FX SN#465-11 pit next to the Delta crane.

29 CFR  1910.303(g)(2)(i):Except as�elsewhere required�or permitted�by this�standard, live�parts of�electric equipment�operating at�50 volts�or more�shall be�guarded against�accidental contact by�use of�approved cabinets�or other�forms of�approved enclosures�or by�any of�the following�means: (A) By location in a room, vault, or similar enclosure that is accessible only to qualified persons; (B) By suitable permanent, substantial partitions or screens so arranged so that only qualified persons will have access to the space within reach of the live parts. Any openings in such partitions or screens shall be so sized and located that persons are not likely to come into accidental contact with the live parts or to bring conducting objects into contact with them; (C) By placement on a suitable balcony, gallery, or platform so elevated and otherwise located as to prevent access by unqualified persons; or (D) By elevation of 2.44 m (8.0 ft) or more above the floor or other working surface.  a) On or about October 24, 2023 at the Giddings and Lewis Mill in the hydraulics area, employees were not protected from electrical of 480 volts due to a cable being cut open and alligator clips being applied. The exposed cable was found on top of a metal cabinet for the hydraulics.

29 CFR  1910.333(a)(1):"Deenergized parts." Live parts to which an employee may be exposed shall be deenergized before the employee works on or near them, unless the employer can demonstrate that deenergizing introduces additional or increased hazards or is infeasible due to equipment design or operational limitations. Live parts that operate at less than 50 volts to ground need not be deenergized if there will be no increased exposure to electrical burns or to explosion due to electric arcs.  a) On or about October 4, 2023 at the Giddings and Lewis Mill hydraulic unit, employees were exposed to live parts when adding oil and working on the hydraulic unit due to a live 480-volt cable being present with the conduit cut open.

29 CFR  1910.147(c)(4)(ii): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following: (A) A specific statement of the intended use of the procedure; (D) Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures.  a) On or about October 3, 2023, the employer did not ensure that the lockout procedure for the Giddings & Lewis Horizontal Boring Mill Model H70FX SN#465-11 provided a statement of intended use for when the lockout procedure was to be used and how to verify that electrical, hydraulic, and pneumatic energy were no longer present.  b) On or about October 3, 2023, the employer did not ensure that the lockout procedure for the Toshiba TUE-200 Vertical Lathe SN 440588 provided a statement of intended use for when the lockout procedure was to be used and how to verify that the electrical energy was no longer present.  The A&B Foundry, Machining and Fabrications and AKA VSCORP, LLC AKA and A&B Foundry LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR  1910.147(c)(4)(ii), which was contained in OSHA inspection number 1482240, citation number 1, item number 3a and was affirmed as a final order on April 26, 2022, with respect to a workplace located at  4754 US 40, Tipp City, OH 45371.
