On or about October 18, 2023, at a jobsite located at 1661 Rush Rd. Wickliffe OH 44092; the employer did not ensure inspections were conducted by a competent person. The depth of the trench was determined to be 6ft 4in. and while installing a sewer line, an employee was exposed to possible cave in hazards due to the lack of cave-in protection.
