29 CFR 1910.178(a)(4):Modifications or additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: (a)Production area: Forklifts, including a Hyster Model H60XT, Serial Number A380V12091V and a Komatsu Model FG45ST-8, Serial Number 131181A, were fitted with slab-handling fork extensions designed to grip, lift and transport stone slabs. This was done without manufacturer's prior written approval. Capacity, operation and maintenance instruction plates, tags or decals were not changed accordingly on or about 10/25/23. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
