29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about October 25, 2023, employees were exposed to a fall hazard of 12-feet while completing roofing work on a residential home located at 4651 East Pleasant Valley Road Independence, OH 44131. Employees were using an extension ladder to access the roof and the employer did not ensure it extended at least 3 feet above the upper landing surface.
