29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   On or about October 25, 2023, employees were exposed to a fall hazard of 13.9-feet while completing roofing work on a residential home located at 7393 Forestwood Drive Independence, OH 44131. Employees were using an extension ladder to access the roof and the employer did not ensure it extended at least 3 feet above the upper landing surface.
