One employee was exposed to CO at an 8-hour TWA concentration of 69.3 ppm during a floor grinding process.

29 CFR 1910.1000(e): Feasible administrative or engineering controls were not determined and implemented to achieve compliance with the limits prescribed in 29 CFR 1910.1000(a) through (d): At 15 College Highway unit C. Southampton, MA: On October 26, 2023, employees engaging in a floor restoration project were admitted to the emergency room and treated for carbon monoxide (CO) poisoning. One employee was exposed to carbon monoxide at an 8-hour TWA concentration of 69.3 ppm and engineering or administrative controls including but not limited to general exhaust ventilation, local exhaust ventilation, or administrative controls were not implemented to reduce worker exposure to CO.

Southampton, MA: On October 26, 2023, and at times prior, the employer did not develop a written HAZCOM program for various hazardous chemicals including but not limited to carbon monoxide (CO) and propane. An employee was exposed to CO at an 8-hour TWA concentration of 69.3 ppm while performing a grinding operation using a propane-fueled Lavina 20G-X floor grinder.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: At 15 College Highway. Southampton, MA: On October 26, 2023, and at times prior, employees were required to use variety of hazardous chemicals while finishing the floor of a facility without being provided effective information and training on the hazardous substances in their work area. One employee was exposed to carbon monoxide at an 8-hour TWA concentration of 69.3 ppm during a floor refinishing process involving a propane-fueled floor grinder.
