29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii) a) New construction of a Holiday Inn located at 1040 Hospitality Drive, Rutledge, GA 30642, Front side: On or about 10/23/2023, and at times prior, the employer exposed employees to fall hazards that exceeded 25-feet to the lower ground level while working from a 7 sections high(6-feet, 6-inches high x 5-feet wide by 7-feet long) tubular welded masonry scaffold system, in that the work platform was not fully planked while engaged in masonry activities.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Cross-braces were used as a means of access. a) New construction of a Holiday Inn located at 1040 Hospitality Drive, Greensboro, GA 30642, Front side: On or about 10/23/2023, and at times prior, the employer exposed employees to fall hazards of approximately 40-feet while working on a tubular welded scaffold system, in that a ladder or other safe means of access was not provided as employees climbed scaffold frames and cross-bracing to access each level. b) New construction of a Holiday Inn located at 1040 Hospitality Drive, Greensboro, GA 30642, Front side: On or about 10/23/2023, and at times prior, the employer exposed employees to fall hazards of over 40-feet while working on a tubular welded scaffold system, in that an employee was working on the cross-bracing.
