29 CFR 1926.102(a)(1): Employees did not use eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents. On or about October 25, 2023, nine roofers were attaching roof shingles with pneumatic nail guns to the roof of an apartment building under construction without the use of eye protection, exposing the roofers to the hazard of being struck-by flying debris or projectiles to the eye(s) or face.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system. On or about October 25, 2023, nine roofers were installing shingles on the roofs of two separate apartment buildings under construction without the use of fall protection, exposing the roofers to the hazard of falling up to approximately 30-feet in height.
