On or about October 20, 2023, the employer failed to report an in-patient hospitalization of its employee who was injured by anhydrous ammonia while performing a line opening on the ammonia refrigeration system.

On or about October 20, 2023, at Labatt Food Services in Houston, TX the contract employer did not ensure that each contract employee follows the safety rules of the facility, including control of hazardous energy during the opening of process lines and equipment. This exposed employees to chemical hazards of anhydrous ammonia.
