At the materials handling yard in 2175 West 18th Street, Jacksonville, Florida, on or about October 25, 2023, the employer exposed employees to trip and slip hazards on the same level, in that the operating conditions at the yard for pedestrian traffic had unsafe and uneven surfaces with numerous potholes.

At the northeast yard in 2175 West 18th Street, Jacksonville, Florida, on or about October 25, 2023, the employer did not ensure the Clark forklift was removed from service until repaired, in that the right rear solid tire was worn-out, cratered with large gouges, and had a 3-inch-wide, 1-1/4-inch-deep hole, exposing employees to a tip-over and struck-by hazard from resulting diminished traction and stability.
