29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. Site- Employees exposed fall hazards while working from a Sky Jack scissor lift and a JLG aerial lift were not provided with operator training. The employees also were not provided training to recognize fall hazards and the procedures to control or minimize fall hazards.

The employees were climbing over the perimeter railing of a Sky Jack scissor lift to access the roof.
