29 CFR 1910.147(d)(5)(i): All potentially hazardous stored or residual energy was not relieved, disconnected, restrained or otherwise rendered safe after the application of lockout or tagout devices to energy isolating devices: At the worksite located at 26530 U.S. 24, Carrollton, Missouri: Employees were exposed burns and pressurized hot material in the form of hot mash when performing maintenance on the flash vessel pump. The employer did not ensure residual energy and material, such as hot mash, in the pipes were relieved, disconnected, or rendered safe after applying energy isolating devices.

29 CFR 1910.147(d)(6): Prior to starting work on machines or equipment that had been locked out or tagged out, the authorized employee did not verify that isolation and de-energization of the machine or equipment had been accomplished: At the worksite located at 26530 U.S. 24, Carrollton, Missouri: Employees were exposed to burns and pressurized hot material in the form of hot mash when performing maintenance on the flash vessel pump. The employer did not ensure piping associated with the flash vessel and flash vessel pump had been isolated and de-energized once energy isolating devices were applied.
