LOCATION: The Derrick Floor above the bus depot at 245 Sumner St.  On or about October 27th:  The employer did not ensure that mesh was installed between the top edge of the guardrail system and the walking/working surface at several locations to prevent loose materials from falling through the existing guardrails.
