29 CFR 1910.157(c)(1):Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries. In the warehouse building, the employer did not ensure employees had clear access to the fire extinguisher exposing employees to fire and smoke hazards. a) Fire extinguisher blocked at the NW wall, b) Fire extinguisher F27 not mounted located on the southwest wall.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure. In the warehouse at the west wall, an extension cord wrapped around a metal conduit and zip tied was used as permanent wiring to energize a floor fan exposing employees to electrical shock hazards.
