Sub-contractor employees engaged in the delivery of materials, that included,  but was not limited to counter tops, kitchen sinks, and bathroom sinks,  to the fifth level of a multi-unit residential building entered the work platform to unload sinks where the platform was not fully secured to the forks and carriage to prevent tipping or becoming dislodged from the forks.  Violation occurred on or about October 28, 2023
