29 CFR 1926.451(c)(2)(v): The employer used forklifts to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied: a. 103-143 E 22nd St., Bayonne, NJ The employer did not ensure that a working platform attached to a JCB, Model 508-66TC, Telescopic Handler was secured to the forks. Sub-contractor employees engaged in the delivery of materials, that included, but was not limited to counter tops, kitchen sinks, and bathroom sinks, to the fifth level of a multi-unit residential building entered the work platform to unload sinks where the platform was not fully secured to the forks and carriage to prevent tipping or becoming dislodged from the forks. Violation occurred on or about October 28, 2023

Sub-contractor employees engaged in the delivery and installation of materials that included, but were not limited to counter tops, bathroom sinks, and kitchen sinks, to the fifth level of a multi-unit residential building worked from an unauthorized work platform. Violation occurred on or about October 28, 2023
