The employer did not ensure that the employees used protective safety glasses, goggles or faceshields, employees were thereby exposed to potential eye injuries.

The lack of proper gloves exposed the employee's hands to severe cuts or lacerations.

The employees were thereby exposed to hazards associated with rotating and moving parts and unexpected start up of conveyor system while employees were standing on the equipment.
